**Diploma/Transcript of Records (TOR)/Copy of Form 138
*Birth Certificate (PSA)
*4 pcs ID Picture, passport size (white background, with collar)
*4 pcs 1×1 ID Picture (white background, with collar)
*1 Long brown envelope
